What Actually Differs
- Crown depth. Some women's-marketed caps are built with slightly shallower crowns, which sit higher on the head. That is a styling choice rather than a sizing one, and it affects how a hat looks more than whether it fits.
- Hair. The largest real variable. Longer hair, volume, braids and ponytails all change how a hat sits and can add a size of effective circumference. This has nothing to do with gender directly and correlates with it enough to matter in practice.

A ponytail or bun sits exactly where a cap's closure does, which means a standard snapback or strapback presses against it or has to be worn above it. Neither is comfortable.
Three solutions exist.

Where the Split Is Worth Making
Having argued that sizing does not split by gender, it is worth naming the two cases where offering a genuine choice does help.
Ponytail-opening blanks. A real construction difference solving a real problem. Offering one alongside a standard cap is the single most effective addition to a mixed-group order.
Crown profile. Some people prefer a lower, closer-fitting cap and others a taller one, and that preference does correlate loosely with how people want a hat to look on them. Offering low and mid profile on one artwork setup addresses it without splitting sizes.
